Requirements for School Entrance



Immunizations

Michigan Law requires that all school-age children be immunized against certain illness. These diseases are preventable if a child is properly immunized against them. The shots that are required are the following:

4 doses DTP+ (a booster if your child’s last shot was received before their 4th birthday)

3 doses OPV+ (a booster if your child’s last shot was received before their 4th birthday)

2 doses MMR

3 doses Hepatitis B

1 dose Varicella (chicken pox vaccine) or reliable history of chickenpox disease.



Central Michigan district health department offers shots several days a week. Please call 773-5921 to make an appointment. You may also schedule immunizations through your private physicians.
